Behind the Screens: The Digital Platform Supporting the Get Healthy Service
CareMonitor celebrates two years since Diabetes Australia was selected in a competitive market process to deliver the Get Healthy Service in partnership with CareMonitor, reinforcing its role as a trusted digital health platform.
The Get Healthy Service is a free, evidence-based phone and online coaching service for people over the age of 16, supporting lifestyle changes and chronic disease prevention. Delivered by Diabetes Australia on behalf of NSW Health, the initiative is supported using CareMonitor’s platform.
Since 1 July 2023, the program has processed over 30,000 patient records and ingested over 150,000 historical records. The platform supported by CareMonitor enables Diabetes Australia’s health coaches to streamline operational processes, collect data efficiently, allowing more time to engage meaningfully with participants.
Powered by CareMonitor’s technology, the Get Healthy Service delivers a NSW population-based health initiative - enabling seamless goal setting, progress tracking, appointment scheduling and communication between participants and health coaches, all through one secure platform.
